By accessing or using this Site or other services, purchasing a sample collection kit, or establishing an OriginalGene account or submitting a sample, we will collect and store your personal information. This information identifies you or can be linked to your identity and account information. The following describes the types of personal information that we collect and store:
Personal identification information
We may collect personal identification information from Users in a variety of ways, including, but not limited to, when Users visit our Site, register on the Site, fill out a form, subscribe to the newsletter, answer survey questions or other communication tools, and in connection with other activities, services, features or resources we make available on our Site. Users may be asked for contact (e.g. name, email address, mailing address, phone number) and payment information (e.g. credit card information), personal information for testing (e.g. date of birth, gender, ethnicity, weight, height, etc.) and information in personal settings. We will collect personal identification information from Users only if they voluntarily submit such information to us. Users can always refuse to supply personally identification information, except that it may prevent them from engaging in certain Site related activities.

a) Full Health (health conditions, carrier status, drug response)

d) Drug (Medicine) Response
e) FIT (diet and exercise)
4. Ancestry Information (genealogical and ethnic)
5. Genetic Information Survey-Based Information (Health Survey and Other Surveys)
Non-personal identification information
We may collect non-personal identification information about Users whenever they interact with our Site. Non-personal identification information may include the browser name, the type of computer and technical information about Users means of connection to our Site, such as language, browser type, the operating system, referring and exit pages, time spent, information searched for, the internet service providers (ISP) and IP address utilized and other similar information.

HOW WE USE COLLECTED INFORMATION
OriginalGene collects and uses your Account Information, Genetic Information, Survey-Based Information, other User provided information and Online Profile Information to operate, provide, analyze and improve our Services.
OriginalGENE’s genotype testing process collects more Genetic (genotype) Information that is used to complete your genetic report that was ordered.
We use your Account Information, Genetic Information, and Survey-Based Information to offer the right products and services from OriginalGene to you. In this case, we maintain control of your personal information.
These activities may include, among other things, using your information in a manner consistent with other commitments in this privacy statement to:
- Your information helps us to more effectively respond to your User service requests and support needs.
- To personalize user experience.
- We may use information in the aggregate to understand how our Users as a group use the services and resources provided on our Site.
- To improve our Site.
- We continually strive to improve our Site offerings based on the information and feedback we receive from you.
- To process transactions.
- We may use the information Users provide about themselves when placing an order only to provide service to that order. We do not share this information with outside parties except to the extent necessary to provide the service.
- To administer a content, promotion, survey or other Site feature.
- To send Users information they agreed to receive about topics we think will be of interest to them.

- The email address Users provide for order processing, will only be used to send them information and updates pertaining to their order. It may also be used to respond to their inquiries, and/or other requests or questions. If User decides to opt-in to our mailing list, they will receive emails that may include company news, updates, related product or service information, etc. If at any time the User would like to unsubscribe from receiving future emails, we include detailed unsubscribe instructions at the bottom of each email or User may contact us via our Site.
HOW WE SHARE THE COLLECTED INFORMATION
Sharing your personal information
We do not sell, trade, or rent Users personal identification information to others. We may share generic aggregated demographic information not linked to any personal identification information regarding visitors and Users with our business partners, trusted affiliates and advertisers for the purposes outlined above.
General service providers.
Since we work with third-party laboratories and contractors to process and analyze your saliva sample for the purposes of generating your genetic report, we share the information described in the previous sections with our service providers, as necessary to provide their services to us and in return to provide the services to you.
Even though our service providers act on our behalf and we implement procedures and contractual terms to protect the confidentiality and security of your information, we cannot guarantee the confidentiality and security of your information due to the inherent risks associated with storing and transmitting data electronically.
OriginalGene will not share your Genetic Information or any other personal information with your employer or health insurance issuer.
Any reseller of our products or services should not collect personal information from you without your consent.
We may use your Genetic Information, Survey-Based Information, and other personal information which will either not include your name or contact information or will be encrypted for internal operations purposes to: validate genetic information; improve the quality and accuracy of OriginalGENE’s genetic reports; develop new genetic reports and/or services, but not publish the results in scientific literature; promote the types of information that customers may learn from our reports; improve the quality and features of our website and services; and produce data for product approval or clearance by FDA or other regulatory authority or body.
We may contact and give you the opportunity to opt-in or refuse before any personal information is shared for any secondary purpose outside OriginalGENE’s immediate control, including published scientific research. We also may pass on to you a research collaborator’s request that you volunteer additional information or participate in a study. No name or Account Information is shared with a research collaborator without your express consent.
We may disclose or report your personal information if the disclosure is required or permitted by law or regulation, a court order or other judicial or legal process or proceeding, or to cooperate with regulators or law enforcement authorities to resolve consumer disputes.

Third party websites
Users may find advertising or other content on our Site that link to the sites and services of our partners, suppliers, advertisers, sponsors, licensors and other third parties. If User chooses to click through to one of the links to third parties’ websites or online services, User must note that any information User provides will be subject to terms and conditions and privacy policies of such third parties. We do not control the content or links that appear on these sites and are not responsible for the practices employed by websites linked to or from our Site and the fact that a link to such websites or services appears in the Site does not mean that we endorse them or approve their policies or practices relating to Users’ information. In addition, these sites or services, including their content and links, may be constantly changing. These sites and services may have their own privacy policies and customer service policies which the User is encouraged to review carefully. Browsing and interaction on any other website, including websites which have a link to our Site, is subject to such website's own terms and policies and Users agree and acknowledge that we will have no liability for any matters relating to third parties’ websites or services.
HOW WE PROTECT YOUR INFORMATION
We have adopted appropriate administrative, physical and technical security measures to protect your personal information against unauthorized access, alteration, disclosure or destruction.
